  • Do you take health insurance?

    No, we do not take health insurance (medicare or third party payers) at this time. Modern Health is a self-pay practice, and patients pay for services at the time of their appointment. 

  • What are your fees?

    Our fee schedule is as follows:


    1. Patient Navigation Services

        $199/hour


    2. Injury and Post Surgical Care

         Initial Consult (Telehealth): Free

         First Visit: $349

         Follow Up Visits: $299 each


    3. Concussion Management

        Baseline Assessment: $99

        Initial Consult (Telehealth): Free

        First Visit: $349

        Follow Up Visits: $299 each


    4. Movement Assessment/Screening

         $99


    5. Failure to Cancel/No Show Fee

         $50


  • What type of payment methods do you accept?

    We accept cash and/or all major credit cards.

  • Do you have a physical location I can go to?

    No, the beauty of Modern Health is that we deliver individualized care to patients in the comfort of their home.

  • How do I get an appointment?

    To inquire about appointments, fill out our Appointment Request Form complete with your contact information and we will contact you in less than 24 hours to schedule an appointment. 


    Appointment times are based on current patient volumes and availability, which vary week to week.

  • What can I expect during my first appointment?

    Your first appointment includes a free, initial telehealth consultation. It will typically last 15-20 minutes and allow us to better understand your injury and course of care to date, so that we may determine if our services are the correct fit for your unique needs.

  • How long will my appointments last?

    All apppintments will last an average of 60 minutes, unless informed otherwise prior to your appointment. 


    Concussion assessments and some baseline testing can run up to 90 minutes depending on each patient's unique circumstances.

  • What should I wear to my appointment(s)?

    Please wear loose comfortable clothing and closed toes shoes to all appointments. We recommend  women to be makeup free with their hair tied back. 

  • How will you navigate the medical system on my behalf?

    As a provider, we are well versed in coordinating medical care, providing complimentary services, and sharing relevant patient information amongst other providers. 


    Our initial focus will be to spend time with you directly so that you understand your injury and why your physician may be recommending certain testing, treatments, etc. 


    Once that is established, we will communicate on your behalf with other providers to ensure all parties are on the same page regarding the necessary care you are recieving and who you are recieving it from.

  • Is there any information I should provide prior to my first appointment?

    Yes, if possible we would prefer to have any and all medical records associated with your injury, so that we may become aware of the course of care up to the point of your first appointment with us.

  • I am already seeing someone else for my condition. Can I still see you?

    In most cases, yes. If we feel that the services we would provide to you are already being providing by someone else, we will recommend you continue with those services.


    However if we determine that your improvement may be expedited with a service we provide, we will recommend a co-treating approach. 


    In many scenarios, we may discuss directly with your other providers about the care you are recieving to determine what approach may work best for you. 

  • How do I get my medical records to you?

    You can email them direclty to us at records@modernhealthsolutions.co or you can request your providers office to send them to us as well.

  • What if I do not have any records from my previous visits/treatments?

    Not a problem. We will do our best to understand your injury and its circumstances during our initial consultation.

  • What if I have to reschedule or cancel my appointment?

    Not a problem. Kindly provide us with more than 24 hours notice of any cancellations.

  • Since Modern Health is a self-pay provider, are you a concierge practice with additional fees?

    No, we do not charge additional fees.

  • I've been told I need surgery. Can you review my case and help me determine if I really need it?

    In many orthopedic cases, surgery should be considered as a last resort. If you see a surgeon, they are likely to recommend it because that is what they do. A major component of Modern Health's practice is in advising clients to pursue treatment avenues that are only medically necessary which, depending on the case, may or may not include surgery.

  • Do you offer discounts for long term care?

    Yes, if you have a condition that requires long term care and you would like to secure a certain number of visits, you can schedule and pay for each of them in advance at a discounted rate. Contact us directly to learn more.
